Illness, disability and life-limiting conditions can have far-ranging consequences beyond an individual’s physical health. Frequently, illnesses can be the consequence of poor medical care borne of a lack of adequate resources (money, health insurance, lack of transportation, family caregiver, etc.). Medical Social Workers can assist in determining patient/family needs and identify any limitations, barriers, or safety issues that might impede recovery. They can educate patients and families regarding community resources to meet identified needs, and assist with long range planning. Specific social work services include:
- Community resource planning, referral service and coordination of services
- Support & assistance in
- Preparation of Power of Attorney for Health Care and/or Living Will
- Coping with emotional adjustments to life changes
- Crisis intervention
- Assistance with applications for financial aid
- Advising with life-style changes
- Grief counseling/coping with impending loss
